Our Commitment to Your Privacy
At NMC Wellness Group, PLLC, we are committed to protecting the privacy and confidentiality of your Protected Health Information (PHI). This Notice explains how we may use and disclose your PHI, your rights regarding that information, and our legal duties under the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy of your PHI, provide this Notice, abide by its terms, and notify you if a breach of unsecured PHI occurs.
How We May Use and Disclose Your PHI
We may use or disclose your PHI for treatment, payment, and healthcare operations. This includes coordinating care with primary care providers, therapists, specialists, pharmacies, laboratories, hospitals, and other healthcare professionals involved in your care; billing your insurance; quality improvement; staff training; compliance activities; and practice administration.
Telehealth and Electronic Communications
We provide in-person and HIPAA-compliant telehealth services. We maintain electronic medical records using secure electronic health record systems. We may contact you by phone, voicemail, secure patient portal, text message, or email regarding appointments, prescription questions, billing, treatment recommendations, or other healthcare matters using your preferred communication method when possible.
Prescription Coordination
As part of medication management services, we may communicate with pharmacies, pharmacy benefit managers, insurance companies, and other providers regarding prescriptions, refill requests, prior authorizations, medication availability, and medication safety.
Business Associates
We work with trusted Business Associates, including electronic health record vendors, billing services, IT providers, accountants, and legal advisors. These entities are required by law and contract to safeguard your PHI.
Uses Requiring Authorization
Except where permitted by law, we will obtain your written authorization before using or disclosing your PHI for marketing, the sale of PHI, or other purposes requiring authorization. If psychotherapy notes are maintained, they receive additional protections under HIPAA.
Uses Permitted Without Authorization
We may disclose PHI when required by law; for public health activities; reporting abuse or neglect; health oversight; judicial proceedings; law enforcement; coroners or medical examiners; workers' compensation; research when permitted; to prevent a serious threat to health or safety; and certain specialized government functions.
Your Rights
You have the right to: request restrictions on certain uses or disclosures; request confidential communications; inspect and obtain copies of your records (subject to limited exceptions); request amendments; receive an accounting of certain disclosures; receive a paper or electronic copy of this Notice; and file a privacy complaint without fear of retaliation.
Changes to this Notice
We reserve the right to revise this Notice at any time. Updated versions will be available in our office, on our website, and upon request.
